REGISTRATION REQUIREMENTS

Please Bring the following information to Central Registration when you come to register:

• Original/Official State Birth Certificate
• Social Security Card
• Comprehensive Physical Examination
• Immunization Record-certified board a licensed physician including blood work and urinary analysis
• Deed or Rental agreement and one additional proof of residency

All the above information must be submitted before the child may be admitted to the classroom. Previous school records/report card may be submitted at the time of registration. Please call the school if you have any questions.
